We are looking for a licensed Physical Therapist to join our compassionate and growing team in the Green Bay area. As a clinician with PRN HHandT, you’ll primarily work in a home health setting, with additional opportunities for cross-training and work in skilled nursing facilities if desired.

You Are a Physical Therapist Who:

• Has great written and verbal communication
• Enjoys building relationships and trust with patients and families
• Works well independently while also collaborating with a supportive team
• Brings compassion, reliability, and clinical skill to every patient interaction
  
  

Job Responsibilities:

• Perform in-home patient evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to improve patient mobility, function, and quality of life
• Collaborate with patients, families, and clinical team members to support care goals
• Accurately document treatments and progress, including discharge summaries
• Supervise and guide Physical Therapist Assistants (PTAs) in treatment plans
• Educate patients and caregivers on safe mobility practices and discharge planning
  
  

Requirements:

• Current Physical Therapist (PT) license in the state of Wisconsin
• Valid driver’s license and reliable transportation
• 1-3 years of Home Health experience preferred but not required—we provide training!
