What You'll Do

As a Registered Nurse Home Health, you will:

• Deliver skilled nursing care aligned with Elara Caring's mission and clinical standards

• Manage a caseload by assessing, planning, coordinating, implementing, and evaluating patient care

• Complete initial and ongoing patient assessments, identifying physical, psychosocial, environmental, and spiritual needs

• Develop and revise individualized plans of care based on patient needs and progress

• Communicate significant findings and changes in condition to physicians, Clinical Supervisors, and interdisciplinary team members

• Conduct follow-up visits based on patient, family, and facility needs

• Ensure patient and staff privacy and confidentiality in compliance with HIPAA

• Collaborate closely with caregivers, families, and the interdisciplinary care team

What is Required?

• Current, unrestricted RN license in the state

• At least one year of nursing experience in a clinical care setting

• Ability and willingness to travel within the branch coverage area (up to 50%)

• Dependable vehicle, valid driver's license, and current auto insurance

• Associate or Bachelor's Degree in Nursing preferred

• Home Health or Hospice experience highly preferred

• Experience with HomeCare HomeBase preferred

• Ability to safely push, pull, lift, and move up to 50 lbs to 100 lbs
