GENERAL SUMMARY:

Henry Ford Health System’s School-Based and Community Health Program provides primary care and behavioral health services to children ages 5-21 regardless of health insurance coverage or ability to pay.
Our clinics are located within more than a dozen schools in Detroit, Lincoln Park, and Macomb County, one community location, as well as a mobile medical unit that travels to various locations.
Provides leadership in the coordination and delivery of quality compassionate patient care.
Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise and document care and teaching of patients and family.
Effectively communicates with the health care team. Demonstrates clinical competence.
RN will be responsible for assisting other RNs with care navigation across Network Project sites (Western High, Earhart Elementary, Maybury Elementary, and Munger Elementary)

EDUCATION/EXPERIENCE REQUIRED:
Graduate from an accredited school of nursing. Bachelor Degree preferred.
Successful completion of HFMG Pharmacology test.
Certification will be required when it is a regulatory requirement in the State of Michigan or a documented community standard, i.e., Advanced Cardiac Life Support (ACLS) or Oncology Certified Nurse (OCN).
Maintains and demonstrates current clinical knowledge in area of specialty.
Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
Ability to perform a wide variety of professional nursing services. Ability to effectively communicate with patients, families, and medical group personnel.
Preferred experience in the specialty.
Basic computer knowledge and keyboarding skills.
3+ years as a RN required
Cross coverage required at other schools nearby if needed.
CERTIFICATIONS/LICENSURES REQUIRED:
- Current licensure by the Michigan State Board of Nursing as a Registered Nurse. Current BLS or ACLS per HFHS policy.
