Key Responsibilities

Collaborate with physicians, nursing, social workers, and support staff to facilitate efficient and safe patient throughput
Conduct comprehensive assessments to identify clinical, psychosocial, and post-acute care needs
Develop individualized discharge plans, including coordination of skilled nursing, rehabilitation, home health, hospice, and durable medical equipment (DME)
Serve as a liaison between patients, families, and community resources, ensuring continuity of care
Advocate for patient rights while balancing utilization review and cost-effective care practices
Monitor length of stay (LOS), readmission risk, and quality metrics
Maintain documentation in compliance with regulatory standards and hospital policy
Communicate clearly with payers to resolve insurance issues and support appeals
Assist in regulatory tasks such as issuing Medicare Important Messages and Non-Coverage Notices

Requirements

Education:

RN: Graduate of an accredited nursing program (BSN preferred)
OR Social Work: Masters in social work (MSW) from an accredited institution

Licensure and Certifications:

RN: Active RN license in New Mexico
Social Work: LMSW, LISW, or LCSW in New Mexico
Case Management certification (ACM, CCM, or RN-BC) preferred
BLS certification required

Experience:

Minimum of 2 years in healthcare, with 1+ year in an acute care setting preferred
Familiarity with discharge planning, medical necessity criteria, and community resources

Skills:

Strong communication, critical thinking, and time-management skills
Ability to multitask and adapt in a fast-paced, patient-first environment
Proficient in EMR and Microsoft Office platforms
