Position Overview

This role involves coordinating healthcare services to optimize patient outcomes through assessment, evaluation, collaboration, and management of care delivery systems. The focus is on implementing cost-effective strategies that ensure high-quality care within the evolving healthcare landscape for the designated patient population.

Key Responsibilities

Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
Coordinate services to maximize patient outcomes effectively.
Assess, evaluate, and collaborate with healthcare teams and participate in service line activities.
Implement cost-efficient care strategies aligned with current healthcare standards.
Manage utilization review, discharge planning, and other healthcare service systems.
Serve as a liaison and facilitator among patients, families, healthcare providers, and insurance personnel.
Oversee direct admission processes and bed assignments for patients admitted through physician offices or transfers from other facilities (emergency department focus).
Provide coverage for holiday shifts as required.

Qualifications

Minimum of three years clinical and case management experience in an acute care environment preferred.
Familiarity with Prospective Payment System, JCAHO standards, and regulatory requirements related to utilization management and discharge planning is desirable.
Strong written and verbal communication skills.

Education

Bachelor's degree in nursing or a related health field preferred.

Licenses

Current Registered Nurse license in California required.
