Responsibilities

As a Hospice Transition Nurse Liaison, you will:

• Represent the branch in various transitional care activities, maintaining relationships with patients, families, physicians, hospitals, and senior living communities to promote Enhabit's hospice services.
• Develop patient-centered transition plans that integrate clinical guidelines and metrics to improve the quality and efficiency of care.
• Act as a public awareness representative by managing educational outreach regarding hospice services and payment sources, particularly the care connections program.

Qualifications

Essential qualifications include:

• Graduation from an accredited school of nursing, therapy, or social work.
• Current state licensure to practice.
• A minimum of two years of clinical experience.
• Familiarity with federal, state, and local laws affecting home health or hospice operations.
• Basic technology skills, including mobile device operation.

Preferred qualifications may include:

• At least three years of clinical experience.
• Experience in home health and hospice practices.
• Understanding of customer dynamics and transitional care best practices.

Requirements

You must possess:

• A valid state driver's license.
• Automobile liability insurance as mandated by law.
• Reliable transportation in good condition.
• The ability to drive safely in all weather conditions.
