Responsibilities
As a Hospice Transition Nurse Liaison, you will:
- Represent the branch in various transitional care activities, maintaining relationships with patients, families, physicians, hospitals, and senior living communities to promote Enhabit's hospice services.
- Develop patient-centered transition plans that integrate clinical guidelines and metrics to improve the quality and efficiency of care.
- Act as a public awareness representative by managing educational outreach regarding hospice services and payment sources, particularly the care connections program.
Qualifications
Essential qualifications include:
- Graduation from an accredited school of nursing, therapy, or social work.
- Current state licensure to practice.
- A minimum of two years of clinical experience.
- Familiarity with federal, state, and local laws affecting home health or hospice operations.
- Basic technology skills, including mobile device operation.
Preferred qualifications may include:
- At least three years of clinical experience.
- Experience in home health and hospice practices.
- Understanding of customer dynamics and transitional care best practices.
Requirements
You must possess:
- A valid state driver's license.
- Automobile liability insurance as mandated by law.
- Reliable transportation in good condition.
- The ability to drive safely in all weather conditions.
