Job description:

Setting: Skilled Nursing Facility, Sub-acute and long-term care patients
Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions
Must be licensed in VT
Experienced in SNF preferred, not required
Potential: 110-225K (Base Salary + RVU Bonus Model)
