Description

• Conduct evaluations of patients in both inpatient and outpatient settings, creating tailored treatment plans
• Deliver physical therapy interventions aiming to enhance mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Instruct patients and their families on therapeutic objectives, discharge protocols, and home exercise routines
• Keep thorough documentation of evaluations, treatments, and patient progress in compliance with facility standards
• Engage in interdisciplinary conferences and care team discussions to guarantee coordinated and supportive care

Education

• Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification strongly preferred
• Additional specialized certifications such as neurological or geriatric are beneficial but not mandatory

Skills

• Prior experience in the field is highly preferred
• Capability to handle patients with complex medical conditions and mobility challenges
• Excellent communication and teamwork skills
