What You'll Do:

• Evaluate patients and develop individualized care plans.

• Provide direct speech-language pathology services in accordance with physician orders.

• Educate patients and caregivers on home therapy programs.

• Ensure accurate documentation and compliance with OASIS and PPS guidelines.

• Collaborate with physicians and interdisciplinary team members for continuity of care.

What We're Looking For:

• Master's Degree in Speech-Language Pathology.

• Current, unrestricted SLP license in the state of practice.

• 1+ year of clinical experience as an SLP.

• Ability to travel within Middlebury and Danbury coverage area (must have reliable transportation, valid driver's license, and auto insurance).

• Comfortable lifting 50-100 lbs and working in a physically demanding environment.
