Description
- Conduct evaluations of patients in both inpatient and outpatient settings and develop tailored treatment plans
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Work collaboratively with nurses, physicians, case managers, and fellow rehab professionals to optimize patient outcomes
- Provide educational support to patients and their families regarding therapeutic objectives, discharge processes, and at-home exercises
- Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with regulatory standards
- Engage in interdisciplinary conferences and care team rounds to ensure well-coordinated patient care
Education
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
- Current state licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (e.g., neurology, geriatrics) are advantageous but not mandatory
Skills
- Prior experience in a clinical setting is strongly desired
- Ability to address the complex medical and mobility challenges presented by patients
- Excellent communication and teamwork abilities
- Familiarity with digital documentation and clinical charting systems
