Description

• Conduct evaluations of patients in both inpatient and outpatient settings and develop tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and fellow rehab professionals to optimize patient outcomes
• Provide educational support to patients and their families regarding therapeutic objectives, discharge processes, and at-home exercises
• Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with regulatory standards
• Engage in interdisciplinary conferences and care team rounds to ensure well-coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurology, geriatrics) are advantageous but not mandatory

Skills

• Prior experience in a clinical setting is strongly desired
• Ability to address the complex medical and mobility challenges presented by patients
• Excellent communication and teamwork abilities
