Description
- Conduct thorough evaluations of patients in a variety of settings, crafting personalized therapy plans
- Implement effective physical therapy interventions that address mobility, strength, balance, and overall function
- Work closely with an interdisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
- Guide patients and their families in understanding therapeutic objectives, discharge plans, and home exercise regimens
- Maintain documentation of evaluations, treatment sessions, and patient progress, adhering to all regulatory and facility guidelines
- Participate actively in care team rounds and interdisciplinary meetings to ensure cohesive patient care
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
- Valid state licensure as a Licensed Physical Therapist
- CPR certification is preferred
- Specialized certifications (e.g., geriatric, neurological) are advantageous, but not required
- Prior experience is strongly preferred
- Expertise in treating patients with intricate medical and mobility challenges
- Exceptional communication and teamwork capabilities
- Proficient in using digital charting and clinical documentation tools
